What is Money Follows the Person?

Money Follows the Person (MFP) is a federal and state demonstration program that helps Medicaid-eligible individuals currently living in long-term care facilities – such as nursing homes, hospitals and other qualified institutions – successfully transition back into the community.

Who is eligible?
People who are interested in living in the community and are eligible for one of Connecticut’s community waivers. To participate in the program, individuals must be Medicaid eligible or want to become Medicaid eligible and are living in a long-term care or hospital setting for at least three months.

How does it work?

Based on individual needs, MFP provides the funds, services and supports needed for a successful transition into the community. In addition, participants can return to their home, or move to an individual apartment, assisted living or group home. A care management team will assess individuals to determine their level of care – including the type and amount of services – individuals will need to be safe in the community.
