A therapeutic recreational program designed for seniors who are experiencing mild to moderate memory impairment. The goal of the program is to help each participant reach and maintain their optimal level of fun
COMMUNITY COMPANION & HOMEMAKING SERVICES – www.communitychs.org
This program provides assistance with in-home services to individuals. Homemakers will assist with light housekeeping, meal preparation, shopping and laundry. Personal Care Attendants will assist with bathing, dressing, grooming and toileting. Services are available 7 days per week.
COMMUNITY HEALTH CENTER, INC. – www.chc1.com
Provides comprehensive dental services in Middletown, New London and Old Saybrook to patients over 60 years of age that would otherwise not be able to afford dental care. The Title III funds allow CHC to improve and meet the healthcare needs of patients through dental care services.
CONNECTICUT LEGAL SERVICES, INC. – www.CTElderLaw.org
Free civil legal representation and counseling to help seniors resolve complex legal issues. Seniors will improve access to medical care, subsistence benefits, preserve their housing and/or protect their housing rights, obtain healthcare directives and Powers of Attorney. CT Legal Services protect them from consumer/finance fraud as well as preserve other elder law benefits to which they are entitled.
DAY KIMBALL HOMEMAKERS – www.daykimball.org
Homemaker and Personal Care
Provides personal care assistance with bathing, dressing, medication reminders, ambulation and other activities of daily living in the clients’ home. Also provided is assistance with housekeeping, shopping and meal preparation. Services are available 24 hours a day, 7 days a week.
ESTUARY TRANSIT DISTRICT – www.9towntransit.com
9 Town Transit provides door-to-door Dial-A-Ride service within the towns of Chester, Clinton, Deep River, Durham, East Haddam, Essex, Haddam, Killingworth, Lyme, Old Lyme, Old Saybrook and Westbrook. Trips may be made for any purpose, including medical appointments, shopping, work, etc. Reservations must be made one day in advance.
MONTVILLE SENIOR CENTER – www.townofmontville.org
Transportation program for all medically necessary services. Available Monday through Friday from 8:00 a.m. to 4:00 p.m. This service provides transportation to doctor appointments, pharmacies, therapy, health screenings, dentists, etc. This is a first come, first served serviced basis.
Routine foot care is provided by a licensed Podiatrist on a monthly basis.
Mental Health Counseling
One-on-one and group mental health services provided by a licensed clinician. This program is available once a week and by appointment or scheduled group time.
NORWICH HOUSING AUTHORITY
The Resident Services Program at Westwood Park and the Dorsey Building provides assistance and up-to-date information regarding medical, financial and social services to meet the needs of an at-risk, economically disadvantaged elderly/disabled.
PLAINFIELD SENIOR CENTER – www.plainfieldct.org
The program will provide transportation for elderly residents of Plainfield (primary), Sterling and Canterbury. This is a door-to-door transport for medical and dental appointments, shopping, social service appointments and social activities.
ROSE CITY SENIOR CENTER – www.norwichct.org/seniors
A benefits counselor is available on average five hours per week. The Benefits Counselor will assist individuals 60 and older from Norwich and its surrounding towns with such programs as Medicare, supplemental insurances, housing, billing, and social security. The Benefits Counselor is able to schedule appointments both at the center and for homebound seniors in the community as well.
A licensed Podiatrist is available for foot care one time per month at the senior center. Transportation to the center is available.

TEEG provides comprehensive benefits counseling to assist senior residents of Pomfret, Thompson, and Woodstock using eligibility screenings in determining their eligibility for federal, state and private assistance programs, Medicare, and DSS benefits in addition to other resources that support seniors in aging successfully at home.

The Falls Talk/Falls Scape Program provides older adults an individualized program to assist adults over 60 years old prevent falls. The program will consist of an initial interview where a customized intervention is developed for the individual utilizing easy-to-use software. To ensure understanding, phone calls will be made to the individual to assess if further training or assistance may be needed. A follow-up interview will be conducted.
Falls Prevention-Physical Activity
Tai Je Quan: Moving for Better Balance provides techniques to improve balance, mobility and daily function in order to prevent falls. This program provides exercises that are designed to improve an individual’s ability to quickly react, so that the individual will be able to catch their balance and be able to prevent a slip or fall.

Adult Day Care
Ross Adult Day Center is a person centered, certified medical model adult day program. Our program is for elderly and disabled adults who live at home and either can’t be left alone for long periods of time or could benefit from increased supervision. Clients come to the center during the day where trained staff provides a range of fulfilling activities, ensuring safety while providing mentally, socially and physically stimulating environment.

Home Health Aide
Home Health Aides provide personal care and assistance with activities of daily living (ADL’s) to individuals who are unable to perform self-care. Activities may include bathing, shampoo, skin care and dressing.
Mental Health Counseling
Mental Health Counseling provides a nurse to make home visits for counseling and medication management to patients with chronic mental illness, depression, and/or Alzheimer’s or dementia. Family support and encouragement to participate in social engagement is included.

Dial-a-Ride provides door-to-door public transportation anywhere within the nine-town Windham Region (Ashford, Chaplin, Columbia, Coventry, Lebanon, Mansfield, Scotland, Willington and Windham). Services are available on a first come, first served bases with a reservation one day in advance.
